Italian number eight Sergio Parisse has been rested for Saturday's match against Ireland, with lock Marco Bortolami captaining the side.
Parisse has been left out in order to prepare for England's visit to Rome next weekend, allowing former captain Bortolami to step in on his 103rd cap.
Robert Barbieri comes in for Parisse at number eight, along with a handful of other changes.
Luciano Orquera and Tito Tebaldi start at half-back for Tommaso Allan and Edoardo Gori, with Joshua Furno moving to blindside flanker.
Another notable change is the return of London Wasps back Andrea Masi, who will wear the number 23 jersey and could win his 81st cap.
Michele Rizzo is back on the bench for the Azzurri after being sent off against France and serving his suspension.
