The Northampton winger had been undergoing the required protocols to return to play, but looks set to lose his race against time to be passed fit.
Scarlets back Liam Williams is likely to take his place on the wing, after missing out on a starting spot for the All Blacks despite his good form.
Wales delayed the naming of their side from Tuesday to Thursday in order to give key players like North, Leigh Halfpenny and Rhys Webb more time.
Halfpenny and Webb are both on track to start against the Springboks, with WalesOnline adding that Gethin Jenkins is also fit enough start at loosehead prop in a real boost.
