The Fijian-born flanker comes into the side in place of Simone Favaro, and will join skipper Sergio Parisse and Alessandro Zanni in the back row.
Vunisa was called up to the squad for the November Tests after impressing for club side Zebre in the Pro12.
That is the only change to the squad from Brunel, with the remaining 22 staying the same after the narrow 20-18 loss to Argentina.
Kelly Haimona continues at fly-half, forming a half-back partnership with Edoardo Gori, while Luca Morisi and Michele Campagnaro continue in the centres.
Martin Castrogiovanni will start as part of an experienced front row alongside Leonardo Ghiraldini and Matias Aguero as Italy look for a first-ever win over the Springboks.
