John Mitchell's USA side hit back to make it 18-8 at half-time with and a second try after the break closed the gap, only for a Lucas Duque penalty to restore a healthy lead at 21-13.
But Niki Kruger's converted try and penalty looked to have stolen the result for the USA after a ten-point swing, before Moises Duque's incredible strike.
The USA remain top of the table after four games, despite their first loss, while Brazil move off the bottom of the table ahead of Chile.
