Hooker Leonardo Ghiraldini and scrum-half Edorardo Gori are both doubtful for Italy's clash with Ireland in Dublin on Saturday.
Ghiraldini injured his shoulder and Gori fractured a rib in the 36-20 defeat to Scotland in Rome in Round Three.
Both players will be given every chance to prove their fitness although Zebre hooker Oliviero Fabiani and Treviso scrum-half Alberto Lucchese have been called up to Italy's squad as cover.
Italy team manager Luigi Troiani said the Azzurri will not rush the decision on Ghiraldini and Gori's fitness.
"We are waiting to evaluate Edoardo Gori and Leonardo Ghiraldini," Troiani told the Six Nations website.
"Gori showed a small rib fracture against Scotland, while with Ghiraldini we are monitoring the progress on an injury to his right shoulder, the trapezius muscle.
"We will wait to assess both players but Oliviero Fabiani and Alberto Lucchese were invited into camp to help us as cover for these injuries."
